Should a deal always be reached according to the exact chip spread numbers provided by the host site? Have you ever even seen a deal reached which goes against those numbers?

You should never take a deal unless it is +EV for you. Most of the time people offering to make deals are playing with scared money so you can use this knowledge to pressure them into giving a better deal that what your actual chip equity is.
Another important thing to keep in mind while making a deal is who is going to be in the big blind next. If you are offered a slightly +EV deal and are going to be the BB in the next hand it makes the deal even better for you since you are about to risk a fair amount of chips.

Have you ever seen a deal that did not come down to the numbers that the host site put forward?

I don't play very many tournaments so I personally have not, but I have heard of it on occasion from friends that play a lot of donkaments.
